Acute sore throat in primary care

1) Problem representation

Adult with acute sore throat, painful swallowing, feverish symptoms, and no obvious lower respiratory features. Consider viral vs streptococcal illness, and screen for complications.

2) Decision aids and key terms

Use scoring tools to structure thinking: FeverPAIN and Centor .

3) Differentials and “must not miss” cues

• Viral pharyngitis (common). Think coryzal symptoms, cough, oral ulcers.
• Streptococcal tonsillitis. Consider exudate, fever, tender anterior nodes.
• Quinsy (peritonsillar abscess) Consider trismus, muffled voice, uvula deviation.

4) Investigations (when relevant)

Many cases are managed clinically. If investigations are discussed, avoid over-interpreting single values: CRP . If you want a short reasoning nudge before deeper exploration: raised CRP .
